It's declining and that's a good thing. Those results are also based on per capita
California still has the most drug users and overdoses. It just has a large population.
There's cities in certain states that a larger overdose counts than West Virginia.
In 2024 and 2025, West Virginia achieved a significant decline in drug overdose deaths, reaching its lowest fatal overdose figures in a decade. Despite this progress, the state continues to have the highest rate of overdose deaths per capita in the U.S. and public health experts are urging continued efforts to address the crisis.

So it's been a problem but the per capita thing per state isn't a good way to look at the worst States. WV only has about 1.7 million people. It had about 700 overdoses and approximately a 40% decline.
WV is making progress. Other States blue ran cities not so much.

As a wvian who loves his state bringing up the **** hole known as Chicago dont change the facts. The only thing that will change wv is to get more middle class jobs. factories up and running will cure a lot of our problems country wide .
I agree WV needs more good paying jobs and investment. Then Training for those jobs. Regardless of the area it's hard to do that when people are abusing drugs and there's crime. So that's got to be delt with everywhere.
So are home State needs to clean certain things up if they want to attract more Investors.

Xylazine is secretly being added to fentanyl and other opioids by street drug manufacturers to intensify the effects of the drugs.
Xylazine is particularly dangerous. It really isn't approved for human consumption. It's actually a veterinary sedative hypnotic,” said St. Elizabeth addiction medicine expert Mike Kalfas.
Kalfas has other concerns too. If there’s an overdose of the drug combination, Narcan will work against the opioid but will not work against xylazine.
“It presents sort of a problem because in an overdose situation, the Narcan isn't going to work. It's not an opiate, so Narcan is not going to reverse it,” Kalfas said.
Since many people are unaware that xylazine is being added, they’re unprepared if there’s an overdose.
