Is there a way to get rid of Pass Interference on the Defense on underthrown balls?
The DB does everything right, ball underthrown, WR comes back and runs into DB, PI on DEF.

It just seems like the DB is in a no win spot.
Offense gets rewarded a first down on a poorly thrown ball.
Would this be too hard to enforce?

Yes. Tell coaches to stop putting DBs in man vs impossible to man WRs.

Bad (or impossible) man D has ruined football at all levels. Send 7, we can go man, just tackle the WR. Embarrassing at all levels.

If he’s playing the ball then he will be absolved of most contact. If he’s not, and he’s impeding the WR from getting back to the ball it’s a foul. Find the ball and you’re off the hook. Other than that how do you define what’s an “under thrown ball”. A back shoulder looks like an under throw. There’s already a fair amount of subjectivity to DPI, I’m not sure you want add an additional level of judgement

If the ball hits the ground it would be live and treated like a fumble. Just like a pitch in the backfield. If it went forward and landed it is an incomplete illegal forward pass. Ball is dead. Enforce from the spot of the pass
