Agree on El Siboney for casual dinner or lunch- a few other favorites:

Breakfast: Blue Heaven, Pepe's

Lunch/Dinner: BO's Fish Wagon (grouper sammich or burger), Hogfish bar and Grill (great casual locals spot), Café Sole' (get the hog snapper),
Santiago's Bodega (great tapas), Café Marquesa, Seven Fish
