Do you think one will be used in our lifetime? (Or again if we have any elders here)

Do you mean by us, or by anyone? I could see some rogue state ruled by a batshite crazy dictator (DPRK) using one, or perhaps a terrorist group like Daesh, but not anyone with any sanity - and that includes some pretty iffy actors like Pakistan.

I think a state--even a rogue one--is highly unlikely to use nukes on a first strike unless they supply a "suitcase" device to a suicide bomber.
