#1 for me: Players hesitating and watching the opposition grab easy rebounds or loose balls.

Still remember Jacob Young hesitating and not getting that rebound miss.

Poor body language after a miss

Not hustling back on defense

Technical fouls for taunting

Not following an instruction after a timeout... ie, calling a TO when instructed that there are none left, or not fouling when told to, etc.

Not making hard quick cuts on inbound plays.

Not finishing plays on defense until the whistle blows.

Not boxing out, especially on foul shots, but in general.

Refs not calling 3 seconds for bigs camping out in the lane, cough cough Purdue.
